What is a CCBHC?
CCBHC stands for Certified Community Behavioral Health Clinic. CCBHCs are not-for-profit companies that can help anyone who walks through the door. A person’s age, insurance status, ability to pay, address and diagnosis do not stop CCBHCs from helping that person.
This type of clinic offers a wide range of behavioral health services. CCBHCs increase access to care and lessen mental health-related hospital stays and arrests. These clinics also improve Oklahoma’s ability to fight the overdose crisis. CREOKS works with schools, hospitals and law enforcement. Together, we create care plans specific to the needs of our clients and the community.
The Oklahoma’s Department of Mental Health and Substance Abuse Services has named CREOKS the CCBHC for eight counties. In some of these counties, we have more than one location. In total, CREOKS currently has eleven CCBHCs sites.

CCBHC Services
			Crisis Services
Crisis services at CREOKS include our mobile crisis hotline and Spring Creek Recovery Centers. These services are available 24/7, every day of the year. Sometimes our thoughts, emotions or behaviors can put us or others in harm’s way. Crisis services are here to help in those emergencies.
			Outpatient Services
CREOKS uses outpatient services to care for our clients without requiring them to be admitted to the hospital. This includes substance use treatment. Outpatient care can be provided in one of our CREOKS clinics. We also make home visits, and for students, we even have school-based services.
			Screening, Assessment & Diagnosis
CREOKS uses questions or tests with clients to pinpoint mental health issues. Our assessment is a deeper evaluation, which could include a medical exam and lab tests. The diagnosis process names the health concern and sets up treatment choices for the client.
			Comprehensive Integrated Care Planning
This type of Care Plan involves a variety of healthcare professionals. We work together to create a plan tailored to a client’s specific needs. For example, we can team up with your primary care doctor to make sure that all of your health needs are being met.
			Target Case Management
Case Managers help you access and understand many health and social services. They will work with you to make sure you get the help you need. This can include local resources like food pantries and clothing closets, etc.
			Peer & Family Support
These staff members are also in recovery from a mental health and/or substance use diagnosis. They are trained in how to support you on your road to recovery. Peer staff can also provide support and resources for families of clients.
			Care Coordination
This service is about treating the whole person, not just the mental side. With your permission, we will work with your other care providers to make sure that all of your health team is on the same page.
			Integrated Primary Care & Screening
CREOKS has a team of primary care and behavioral health providers who work together for screening and delivering care that addresses mental health, substance use and other medical needs.
			Psychiatric Rehabilitation
The goal of this service is to give clients hope, motivation and skills so they can be more successful in society. Psychiatric Rehabilitation can be one-on-one or in a support group.
			Housing & Employment
CREOKS uses evidence based practices to help clients find jobs and housing. We know that a safe place to live and a sense of purpose are essential parts of mental and physical wellbeing.
			Veterans Services
We have a team of staff who are educated in military culture and the unique challenges faced by veterans. Some of these staff have served in the military themselves. Our veterans team works with the VA to meet the needs of Oklahoma service members.
			Outreach
CCBHCs must provide care where it is most needed, and outreach efforts mean we are able to serve more clients. We have staff who focus on outreach and engagement activities. The goal is to support access for underserved and vulnerable populations.
