Aging services
CREOKS Living Better, Longer is a specialized program for adults 65+ and adults 50+ with complex medical or mental health needs. Services are available in person and virtually and support physical, mental, emotional, and social well-being. Our goal is to help you age positively, stay connected, and maintain the highest possible quality of life. Through personalized care, engaging activities, and social support, we help you build the skills and connections needed to thrive.
Services include individual and group therapy, case management, medication management, peer support, and coordination with your primary care provider and other healthcare professionals. We also support family members and caregivers while addressing concerns such as depression, anxiety, trauma, cognitive decline, diabetes management, and thoughts of self-harm. Our goal is to help you maintain independence and live well at every stage of aging.

CREOKS: LIVING BETTER, LONGER SERVICES
Aging brings changes for all of us, and we are here to help you navigate them. Just as health education supports healthy choices, understanding aging and mental health can help create a fulfilling and meaningful life as an older adult.
CREOKS has made older adult care a priority, partnering with nationally recognized leaders in aging services, including the E4 Center at Rush University and the University of Washington’s PEARLS (Program to Encourage Active, Rewarding Lives) program. Our team has received specialized training to better support the unique needs of older adults.
Our goal is to get to know you, partner with you, and create a personalized treatment plan that supports your independence, quality of life, dignity, and overall well-being.
Individual & Group Therapy
Therapy can be a helpful support during times of change. In individual or group sessions, you’ll have a safe space to talk through challenges and connect with others.
Case Management
Case management helps you navigate health and social services. We collaborate with your medical team and community partners like Meals on Wheels and the Area Agency on Aging to build a coordinated support system for you and your family.
Medication Management
Our team, including a nurse, reviews your health situation and works with your medical providers to help identify and address any concerns.
Peer Support
Peer support connects you with an advocate and friend who is also navigating aging. You can learn from their experiences and share your own.
Caregiver Support
CREOKS provides support and resources for caregivers, individuals, and families caring for a loved one through aging. Caregivers may also be eligible for services to support their own well-being.
Education
CREOKS provides education on aging research, medical advancements, and strategies through our Living Better, Longer program.
CREOKS: LIVING BETTER, LONGER TEAM
All members of the Aging Services Team have completed specialized training, including the 4Ms model (What Matters, Medication, Mentation, Mobility) and the PEARLS model (Program to Encourage Active, Rewarding Lives). This training helps the team provide compassionate, culturally responsive care that supports older adults and their families.
A. Ann Wilson
Director of Aging Services │ Therapist
Ann earned a bachelor’s degree in accounting and worked as a CPA before obtaining her master’s degree in counseling in 1994. A Licensed Professional Counselor since 1997, she has extensive experience serving clients of all ages in crisis care, inpatient, outpatient, and private practice settings. Ann is passionate about evidence-based care, interdisciplinary teamwork, and supporting both individuals and families affected by mental illness. She helped develop CREOKS’ Aging Services Program alongside Chris Flanagan, Jonathan Pierce, and CREOKS leadership. Her guiding belief is that every person deserves dignity, respect, and compassionate support through life’s challenges.
THe 4ms Model
The CREOKS Living Better, Longer program is dedicated to providing the highest quality of care through our components from nationally recognized programs. This specialized model would be categorized as an “evidence-based” model, which means it has been researched and proven to be effective with treatment for older adults. We are recipients of a grant from the E4 Center at Rush University Center for Excellence in Aging, and our approach is built on their core principles:
1. What Matters: What Matters to YOU: the things you value, the goals you have for your life, your ethnic, cultural, and individual preferences are at the center of every service.
2. Mentation: We support your mental and cognitive health and help you identify ways to stay alert and learning.
3. Mobility: We help you learn ways to stay as active and safe as possible, ways to maintain your independence, ways to identify risk factors and prevent falls while remaining as mobile as possible.
4. Medications: We review your medications and medical needs and collaborate with your medical team with the goal of maximum health.
Caring for Caregivers Model
As a supplement to the 4Ms model, the CREOKS team will involve individuals, family members and caregivers as you designate. The needs of the people that support you can be assessed, and they can be involved in our services as you specify. Resources will be identified and often services are offered to them separately from yours if that is appropriate.
Pearls Program
The PEARLS (Program to Encourage Active and Rewarding Lives) program is also an evidence based treatment program for older adults. The goal is to assist people dealing with depression who have become less active, do fewer things they enjoy and feel overwhelmed by the circumstances in their lives.
This treatment program helps you learn ways to identify fun things to pursue, develop physical activities that fit your health needs and reduce symptoms of depression to improve your quality of life. PEARLS empowers you through behavioral techniques to actively manage depression. A trained PEARLS Coach will meet with you for 6-8 sessions, which can be held over the phone, on a computer or in-person. You will learn the skills necessary to make lasting changes.
